The FMS has the following basic requirements for how to correctly fill out the application form for a new international passport:

How to fill out a form for an adult

An application for a passport for an adult consists of 14 points. The first page contains basic personal information:

  1. Last name, first name and patronymic are indicated in Russian as they are written in the citizen’s passport.
  2. Gender is selected from two options in accordance with what is indicated in the passport, regardless of which gender the applicant considers himself to be.
  3. The date of birth is indicated in full in the format dd.mm.yyyy (for example, 01/01/1980 - January 1, 1980).
  4. The place of birth is written completely in accordance with the passport data.
  5. Information about your previous last name, first name, patronymic and gender. If none of this has changed, then you need to mark the answer “No”; if changes have been made, then indicate “Yes” and enter all the information. It is worth noting that if this data has changed more than once, then the application itself indicates only the last change of last name, first name, patronymic, and all previous ones are entered on a separate form, which can be downloaded on the official website of the FMS.
  6. The address of the place of residence is indicated in full in accordance with the registration stamp in the passport, and the date of registration is also entered.

The second page of the questionnaire is devoted to the applicant’s work activity and has a number of features for filling out:

  1. The first date is set exactly 10 years before the date of filling out the application, regardless of the fact that the person was previously at this place of work or study. For example, the applicant studied at the university from September 2002 to June 2007, and she filled out the application in December 2014. Accordingly, the first entry will be dated December 2004, despite the fact that the training began two years earlier.
  2. If a person studied and worked at the same time, then both places must be entered in the application form: work and study.
  3. If for more than 3 months the applicant did not study, serve or work, then this period must be entered into the table. In this case, in the column “Position and place of work, number of military unit”, “Did not work” is indicated, and in the column “Address of the organization, military unit” the home address is entered.
  4. The last entry must contain the inscription “N.V.” in the “Date of Dismissal” column. (present tense).

How to fill out a form for a child under 18 years of age

For a child, you can fill out the application form for a new type of international passport in the same way as the application form for an adult. On the first page information about the minor himself is entered:

  1. Last name, first name and patronymic in accordance with the main document (birth certificate for children under 14 years of age and passport for teenagers older).
  2. Gender of the child.
  3. Date of birth in the format dd.mm.yyyy.
  4. Place of birth (as in the main document).
  5. Previous last name, first name, patronymic, if they were changed. If the data has changed several times, you must fill out a separate form with additional information.
  6. The child’s place of residence (“registration”) and the date of registration.
  7. Address of the place of stay or actual place of residence of the child. If there is registration at the place of stay, its validity period should be indicated.
  8. Personal identification document details: series and number, by whom and when issued.
  9. Foreign passport details, if its validity has not yet expired.

The child’s signature is placed when submitting documents and only if he is already 14 years old. A sample of filling out the first page of the application is shown in the figure.

The second page of the application form must contain information about the child’s legal representative (parent, adoptive parent, guardian, trustee) who will accompany the minor when submitting documents. You must provide information similar to the first page:

  1. Last name, first name and patronymic of the legal representative in accordance with the passport.
  2. His gender.
  3. Date of birth of the parent (adoptive parent) or guardian (trustee).
  4. Place of his birth.
  5. Address of residence in accordance with the stamp in the passport and date of registration.
  6. Address of place of stay or actual place of residence.

How long does it take to get a foreign passport?

The classic time period for producing a document, if a package of papers was submitted at the place of residence, is 1 month.
