Taking a spermogram is necessary to identify the causes of possible infertility in men for couples who do not have children.

Many families want to have offspring, are examined to find out why there is no pregnancy. The wife does it first, if nothing is found it is the man's turn.

But often he does not want to examine the semen, he prefers to leave everything as it is. Today we'll talk about what it is, how to take a spermogram.

What is the analysis for

Infertility is common these days. 50% of cases - men are to blame. In addition to pathological processes in the body, the cause of infertility is the modern pace of life - stress, chronic fatigue, venereal diseases, ecology.

Sperm analysis is needed to determine the quality of the semen, the mobility of the gum, the ability to connect with the egg. Not always a bad result indicates infertility.

The composition of the semen can change over a short time, the reasons are different.

Taking a spermogram will allow to determine not only the possibility of a man to conceive, but also to identify diseases of the genitourinary system. Additional examinations will help establish an accurate diagnosis and begin timely treatment.

In what cases it is taken:

  • The main reason for which he is appointed is the inability of the couple to have children. If for a long period of 1-1.5 years the spouse cannot become pregnant, the andrologist recommends that both of them be examined.
  • The main test that a man does is a spermogram. If young people cannot conceive a child naturally, IVF is prescribed.
  • The analysis is given to determine the pathology of the male reproductive system. Its indicators will help establish your overall health.

Training

  1. Alcohol and nicotine negatively affect the quality of semen. 7 days before the delivery of the ejaculate, alcohol must be completely excluded, no smoking.
  2. Rational nutrition has a beneficial effect on the male body. Refrain from spicy, salty foods. Do not eat foods containing chemical elements (canned food, pates, stews).
  3. The temperature required for the maturation of gametes is 33-35 degrees. Its increase has a negative effect on spermatogenesis, so you can not go to the sauna, baths, take hot baths.
  4. Leading a hectic life, constant stress, negative emotions - all this negatively affects reproductive function.
  5. Tight synthetic clothing is not suitable for men. The blood flow to the genitals is blocked, the temperature rises - this is unhealthy, it causes diseases of the reproductive system.
  6. Medicines affect the test result. For a week, you need to stop taking them, you should consult a doctor when you can not stop drinking, it is required (for some diseases).
  7. You need to refrain from sexual intercourse for 5-7 days. A week is the optimal time for the accumulation of ejaculate in the required quantities, substances are accumulated that will help to accurately analyze, determine the ailments that have arisen.
  8. It is not recommended to donate sperm for colds, infections of the genitourinary system. It is necessary that at least 14 days have passed since the illness.

If the rules for passing the spermogram were followed, but the result shows deviations from the norm, do not despair. The initial analysis does not reflect the real picture of the disease, it cannot be final, it is necessary to continue the research.

Having prepared, retake in two or three weeks. This result will also not be final. For a more accurate determination of deviations in the analysis, it is necessary at least three times, the average total is displayed.

How to take a spermogram

For reliability, it is better to come to the clinic. At home, the analysis is not always sterile.
And also due to the fact that it was delivered to the laboratory with a delay, one of the indicators cannot be determined - liquefaction.

To take a spermogram at home, you must observe the following rules:

  • There should be sterile utensils, preferably taken from the clinic. It is strictly forbidden to collect semen into a condom, its lubrication adversely affects the ejaculate.
  • It is necessary to deliver the biomaterial to the clinic within 45-60 minutes to determine the liquefaction.
  • During transportation, it should not be shaken strongly, the temperature regime must be observed, as this can lead to distortion of the results.

Clinic

There are rules that are mandatory for the reliability of the analysis.

  • First, a man needs to wash his hands and penis with soap and water. This is necessary to maintain hygiene when delivering biomaterial.
  • The cabinet in which the process takes place must be well heated. This is important for the accuracy of the result. At low temperatures, the composition of the ejaculate will be disturbed. Optimal 18-20 degrees.
  • How long does the procedure take? For this, a separate room is provided, which can be closed from the inside, so that the patient feels free. The time is not limited.
  • How is the analysis taken? Ejaculate is obtained by masturbation. Other forms are not acceptable. The man is in the office himself.
  • What do you need for this? Semen collection should be done in a sterile plastic or glass container provided by the clinic.
  • If you can't get tested on your own? Special literature, musical accompaniment, video that helps a man to relax is used.
  • The container must be signed. Full name, time, period of abstinence, amount of fluid. If part of the seed was lost during collection, this should be indicated on the container.

In ejaculate, it is not the quantity of livestock that matters, but their quality. To establish the morphology of spermatozoa, in addition to the spermogram, a Kruger test is made.

It includes the following indicators:

  1. How much fluid comes out during ejaculation.
  2. How many gametes are there in 1 ml, the total number in the ejaculate.
  3. Kruger appearance.
  4. Mobility.
  5. Caking test.
  6. Other elements of the semen.
  7. Chemical composition.

The morphology score averages 30-87%. Sometimes 15% of live sperm is allowed, which is enough for fertilization.

Where to rent

Private clinics are equipped with modern offices, comfortable conditions, the ability to quickly and efficiently make an analysis.

Conventional laboratories are not suitable for delivery, ejaculate can be brought there from home, but there is a possibility of distortion, since the spermogram must be transported correctly.

If the first result does not correspond to the norm, it is recommended to repeat it 3-4 times within 2 months, preferably in different laboratories.

A correctly passed analysis will help you understand the situation faster. We need to be patient to pass this test. Read new articles about men's health on our website.

The spermogram for analysis is carried out in a special laboratory under sterile conditions. The sperm is collected in a test tube after masturbation. Be sure to wash the genitals and hands before manipulation. It is better to donate sperm in a clinic so that it can be immediately examined. For this, special rooms or offices are organized, although such conditions are not everywhere.

Sometimes it is allowed to collect an analysis at home, observing the rules of asepsis, but since the result is often unreliable, since it takes time for delivery or it is not possible to provide a temperature regime, it is better to take a spermogram in a laboratory.

How to prepare for delivery

For the result to be accurate, you need to adhere to a few simple rules:

  • Sexual abstinence is required for 3-5 days.
  • During this period, you should not overheat, visit baths and saunas.
  • You can not drink alcohol, even beer and some medicines, if you are taking any medications, warn your doctor.

Where to get tested?

Patients are often worried: "Where can a spermogram be done?" In Moscow, many medical institutions offer this service, but, of course, you need to choose a clinic with a well-equipped laboratory and highly qualified specialists. Since an incorrectly performed analysis can give an inaccurate result, and then you will need to take a spermogram again. Often the reason for the need for re-analysis is the incorrect preparation of the patient.

Semen analysis should be performed by a trained and trained specialist, since some indicators may not be noticed immediately, and some work experience is required. The main indicators are the number, motility and appearance of spermatozoa, it is these indicators that determine its quality.

Sperm analysis in Moscow can be done at the AltraVita clinic. We have created all the conditions that provide comfort: a separate room and auxiliary materials. Our specialists have extensive experience in the diagnosis and treatment of male infertility. Every day we donate sperm for various types of examination, including extended. How much does a semen analysis cost, you can find out on our website or by calling the reception.

Sperm indicators

Sperm count

The number of sperm cells in the ejaculate is normally more than 15 million / ml. When fewer of them are found, then this is oligozoospermia, and it is the cause of difficulties with conception. When there are no sperm in the ejaculate, this is azoospermia, and, therefore, conception with such an indicator is impossible.

Sperm activity

A normally developed sperm cell moves actively and in a straight line. By examining the sperm, the doctor counts the number of viable spermatozoa, selects those that move in a straight line. If the sperm is inactive, oscillates or rotates in a small area, then such sperm cannot go all the way from the vagina to the fallopian tube to participate in fertilization. A large number of these sperm prevents normal, straight-moving sperm from reaching their target. The diagnosis of such a pathology is asthenozoospermia.

An effect such as agglutination can also interfere with normal movement. This is clumping, gluing of sperm with the formation of conglomerates. Naturally, stuck together spermatozoa also become restricted in movement and cannot participate in fertilization.

Quality

The shape of the sperm is also important. A normal sperm cell has an oval head with an acrosome, a neck, and a long, convoluted tail. If the external structure of the sperm does not correspond to the norm, then such sperm is most often inactive or immobile. Thus, nature makes sure that mature sperm with a normal set of chromosomes and no defects participate in fertilization.

Examining a sperm sample, the doctor counts sperm of irregular shape and those that have no acrosome, no tail or 2-3 of them, irregular head shape. Such sperm can be found in the semen of any man, but if their number exceeds the norm, then the diagnosis is terratozoospermia.

The state of DNA plays an important role in determining the quality of sperm. Sometimes, during examination, you can find that the DNA in the nucleus is unevenly colored and consists, as it were, of small fragments. This kind or fragmented DNA is a pathology that must be taken into account, because an egg fertilized with such sperm can receive low-quality genetic material, which will lead to its death or malformations in the fetus.

If the above pathologies are absent in the spermogram, then normozoospermia is written in the doctor's conclusion, which indicates good sperm quality.

Sperm morphology

Sperm quality is determined in two ways:

  • The sperm is examined under strong magnification in order to identify violations of the shape of the head, its size. This study is carried out during a regular spermogram, it does not accurately determine the pathologies of the development and structure of sperm, therefore, the norm with such a count of high-quality sperm is the presence of more than 50% of outwardly normal-looking sperm.
  • For a more accurate determination of structural pathologies, the Kruger method is used. According to Kruger, the ejaculate of a healthy man should contain at least 13% of morphologically normal spermatozoa. That is, many more men can be considered healthy.

The Kruger method requires a more thorough examination of the morphology of the sperm, for this a smear is made on glass and it is stained using a special technology. This allows, under high magnification, to see the finer details of the cells, the uniformity of the nucleus, the correct size and location of the acrosome.

This is a deep and complex study that requires additional smear preparation. Therefore, if patients ask: "How much does it cost to take a spermogram analysis according to Kruger?" - it should be noted that such a study is somewhat more expensive, because it is performed only in specialized centers for the treatment of infertility, which have high-quality, expensive equipment and specialists specially trained in this matter.

With this method, sperm is examined more reliably, and the cost of sperm analysis is higher. The AltraVita clinic is one of the few in Moscow where this technique is used.

Reviews of patients who underwent a spermogram with Kruger morphology are mostly positive, since this analysis made it possible to revise the treatment plan and achieve good results.

Additional indicators

When studying the spermogram, the doctor must pay attention to the cells that practically should not occur, these are leukocytes and erythrocytes. They can be seen only after special staining, since without staining the preparation it is difficult to distinguish them from immature sperm. The presence of leukocytes in semen is an important diagnostic sign that can indicate the cause of infertility. Its rate is 1 million / ml. An increased content indicates inflammation and requires additional research, especially since the inflammatory process usually has a negative effect on spermatogenesis.

Other indicators of the spermogram, which are determined during the analysis, are as follows:

  • Volume. The required volume for fertilization is considered to be 3-4 ml, although when the spermogram is submitted, it can be 1-6 ml. This indicator may depend on the time of abstinence, nutritional quality and some other factors. But oligospermia is also found in some diseases, for example, retrograde ejaculation.
  • Color. Healthy sperm can be grayish white, slightly opalescent. The acquisition of various other shades by her is associated with the intake of certain medications, bleeding in various parts of the genital organs and infections.
  • Smell. Normally, semen has a slightly alkaline odor; a change in it can also indicate a change in metabolism and infection. Therefore, if an unpleasant specific odor is detected, a thorough examination is required.
  • Viscosity. This is an important indicator that can be the cause of infertility. The fact is that sperm initially has a certain viscosity, when it enters the genital tract of a woman, it becomes thicker at first, and then liquefies again. This is necessary to initially protect the sperm from the aggressive environment of the vagina, and then help them move along the genital tract to the ampullary tube. Semen that is too thick is a sign of endocrine disorders. Sperm cannot actively move in it and therefore difficulties with conception arise.
  • Acidity. It may depend on the general condition of the body and concomitant diseases. Its violation leads to the death of sperm cells, which are not adapted to abrupt changes in pH.

The results of the examination are reviewed by the attending urologist and the appropriate treatment is prescribed based on them. If the indicators do not correspond to the norm, and there is a suspicion that the analysis was taken incorrectly, then it is recommended to re-pass the examination no earlier than 2-3 weeks later, observing all the conditions for the delivery of the biomaterial and preliminary preparation.

It should be borne in mind that the quality of sperm can be influenced by:

  • somatic diseases;
  • infectious processes;
  • overwork;
  • improper nutrition;
  • bad habits (smoking, alcohol and drugs);
  • stress;
  • taking certain medications.
Other examinations for male infertility

Additional semen tests may be needed if abnormalities are found during the study. For example, in case of agglutination, an additional MAR test is performed; with oligospermia, an ultrasound scan is required; Vasography, detection of white blood cells, and foul odor require testing for STDs.

Antisperm antibody test (MAR test)

Antisperm antibodies (ATs) are protein compounds that bind to receptors on germ cells, thereby reducing their mobility. Sperm cells are combined into conglomerates of various sizes. This phenomenon is called agglutination, and the higher it is, the lower the chances of conceiving a child.

Antisperm antibodies can be produced in both women and men, they are determined using the MAR test. In men, blood and semen are examined, in women - blood and mucus from the cervix. The test is considered negative if less than half of the sperm are involved in agglutination. This test is the main method for detecting immunological infertility.

Screening for STDs

Inflammatory diseases of the genital tract caused by infections lead to impaired spermatogenesis and sperm maturation. A large number of altered forms of sperm (terratozoospermia) can appear in the semen. It is possible to identify the causes of violations using a number of PCR tests, which will show the presence in the body of infections such as chlamydia and herpes.

Bacterial sperm culture with antibiotic susceptibility testing

It is prescribed when a large number of leukocytes are detected in the sample. The preparation and sampling technique is the same as for a conventional spermogram. You can not use antibiotics two weeks before the test.

Ultrasound

It is performed transrectally, it allows you to study the prostate gland, testicles and their appendages. With the help of ultrasound, mechanical blockage of the seminal ducts and disturbance of blood flow in the genitals (venous reflux and varicocele) can be detected.

Vasography

This is a study aimed at detecting obstruction or narrowing of the ducts of the seminal vesicles. Such abnormalities can cause sperm congestion and impairment of spermatogenesis.

Genetic analyzes

They are prescribed for suspected hereditary infertility. In some cases, pathologies of genes and chromosomes can cause a violation of human reproductive function. In order to identify the most common genetic disorders, karyotyping is done. Thus, changes in the AZF region of the Y chromosome can be a sign of male infertility and impaired spermatogenesis.

Testicular biopsy

This is a necessary intervention that can confirm absolute infertility in men. It is carried out in cases of azoospermia, when the question is being decided whether it is possible to restore the function of the testes or whether it is necessary to solve the problem of infertility by using donor sperm. This procedure is carried out only in large, specialized medical institutions, including the clinic "AltraVita".

How much does a spermogram cost in Moscow?

Most patients want to have a cheap spermogram in Moscow. I must say that prices differ in different institutions. In some clinics, the analysis can be done inexpensively, in others the prices are very high. They depend on different indicators. And even in the same clinic, the cost depends on whether extended studies, for example, Kruger morphology or MAR test, will be carried out. In addition, in some cases, an additional study may be prescribed, therefore, how much it costs to take a spermogram in Moscow and the prices for it should be checked with the medical staff.

Contact the AltraVita clinic if you urgently need to take a spermogram in Moscow. Results will be ready in a few hours.

Biomaterial "sperm" for analysis "Spermogram" is accepted ONLY in laboratory departments in Moscow and the Moscow region, equipped with semen collection rooms, by appointment.

Biomaterial brought with you in sterile containers is not accepted!

Spermogram - This is an analysis of semen (ejaculate), necessary to determine male fertility (ability to fertilize) and identify possible diseases of the reproductive system. A spermogram is prescribed when a married couple complains of infertility; a spermogram is also indicated for sperm donors and people planning sperm cryopreservation.

It is done if:

  • there is a suspicion of male infertility;
  • direct treatment for infertility is underway;
  • preparation for in vitro fertilization (IVF) is underway;
  • preparations for intracytoplasmic sperm injection (ICSI) are underway;
  • a man wishes to undergo sperm cryopreservation or become a sperm donor.

A standard spermogram takes into account physical parameters: semen volume, color, viscosity, pH; and microscopic parameters: the number and motility of sperm, the content of other cells, etc. Based on the data obtained, an assumption can be made about male infertility, prostatitis and possible infections. It is incorrect to evaluate the fertilizing capacity of sperm by individual parameters; it is necessary to take into account all indicators at the same time. Even in the same man, sperm counts can vary significantly throughout the year. When diagnosing male infertility with results that differ from the norm, it is necessary to retake this study after 1 - 2 weeks and, with the results obtained, consult a doctor to find the causes of the pathology that has arisen. When re-examining, it is advisable to adhere, if possible, to the same periods of abstinence in order to correctly assess the results obtained in dynamics.

The price of a spermogram is only 1,480 rubles. The time frame for the standard analysis is 1 day. The Hemotest Laboratory has a flexible system of discounts up to 15%!
